Please include the following information:

1. Maiden name
2. Year of Graduation
3. Birthdate
4. Name and address where to send the transcript

Official transcripts can only be sent to colleges or places of employment. If a student needs a transcript, a Student Transcript can be mailed.

If verification of graduation is needed, a Diploma Letter can be issued by following the same instructions as if requesting a transcript.
